Your employer cares about you and your family, and would like to assess your daycare needs.  By daycare we mean any arrangement in which your child/children spend part of their day with someone other than a parent or guardian (when they are not in school). The highlighted (green) statements below are linked to existing web sites that will ask a series of questions designed for your specific form of daycare.  Please select the one that most closely resembles the daycare arrangements you use for one or more of your children (don't worry if the one you choose does not apply to all your children--simply do your best!).  Your assistance is appreciated and will aide your company in assessing your daycare needs.  Please select one of the following options below:
